Carson City man jailed for allegedly forging prescription from doctor

A 41-year-old Carson City man was arrested Thursday, 4:32 p.m. in the 500 block of East William Street on felony suspicion of forgery, a Carson City sheriff's deputy said.

Robert Wayne Smith was jailed on $5,000 bail. Officers spoke with the pharmacist who received a call from a local doctor who asked the pharmacist not to fill any new prescriptions for the man as they were all forged, the arrest report states.

Officers verified the signature of the prescription and that from the doctor, which had a different signature then the verified one. The man later admitted that he had forged the prescription. He was taking into custody for uttering a forged instrument, a felony. Bail: $5,000.

In other arrests: A 34-year-old Carson City woman was arrested early Friday, 2:01 a.m. in the area of Carson Meadows and Como streets on two gross misdemeanor and five misdemeanor charges.

According to the arrest report, officers with the Carson City Sheriff's Office Special Enforcement Team were patrolling the area on Musser Street and saw a vehicle swerve and then continue into the center lane for several feet.

The vehicle did not stop at a stop sign and the officer paced the vehicle going 35 mph in a posted 25 mph zone. The officer attempted to stop the vehicle on Musser and Carson Meadows. The vehicle came to a stop at Carson Meadows and Como Street.

The officer spoke with the woman who provided a license. She had a smell of alcohol on her breath. The officer asked her how much she had to drink. The woman advised one beer.

Officers asked her to submit to field sobriety tests where the woman complied but did not complete the tests satisfactorily. She submitted to a preliminary breath test with a .104 on a partial blow. She was placed into handcuffs and in the back of a patrol vehicle.

The woman became verbal, screaming out of the window of the patrol vehicle. When officers arrived at jail, the woman kicked an officer in his left knee.

Officer searched the purse of the woman and located a marijuana pipe, four grams of marijuana, 11 Zolpidem Tartrate pills and 1 Cyclobenzaprine pill in her purse. The pills were not in a prescription bottle and were mixed in with the marijuana, the report states.
Because of the woman's refusal to comply with consent laws, officers contacted the on-call judge and district attorney requesting a search warrant for blood evidence. The judge granted the search warrant at 2:50 a.m.
At 4:01 a.m. a blood sample was taken. The woman faces the following charges: Gross misdemeanor possession of dangerous drugs without a prescription and battery on a police officer. She faces the following misdemeanor charges: Failure to stop at a stop sign, driving left of the center lane, possession of marijuana, possession of drug paraphernalia, DUI alcohol and drugs. Bail: $8,225.

— A 19-year-old Carson City man was arrested July 4, 12:53 a.m. in the 1800 block of North Carson Street on misdemeanor charges of warrant failure to appear, driving on a suspended license and driving without headlights. Bail: $1,124.

— A 23-year-old Carson City man was arrested early Friday morning, 2:43 a.m. in the 400 block of North Carson Street on a gross misdemeanor charge of being in possession of a dangerous weapon.

According to the arrest report, officers were dispatched to a casino in reference to a fight involving three subjects near the parking lot area. Officers arrived on the scene and found security had one man in custody. He was cuffed and checked for weapons.

As he was being cuffed he was asked if he had any weapons on him. The man replied he had a switch blade knife in his left front pants pocket. Officers removed the knife, approximately 6 inches in length. The knife had a spring loaded button on it, and when the button was pushed, a metal blade approximately 3 inches in length appears. He was arrested on the charge of possession a dangerous weapon. Bail: $2,500.

— A 22-year-old Carson City man was arrested Wednesday, July 2, 7:15 p.m. on three misdemeanor charges including petit larceny and possession of stolen property. According to the arrest report, officers responded the 1900 block of Highway 50 East regarding a stolen iPhone.

A man, who had been loitering in the area, approached the officers and asked if he could lock his bike to a fire hydrant. Officers asked the man if he had found a missing cell phone in the parking lot.

The man told officers he recently purchased an iPhone for a man for $100. He stated the man he bought the phone from made an arrangement to pick up the phone on a movie kiosk box at a nearby gas station.

The man told officers no one was around when he picked up the iPhone that he said had paid $100 for. He did not know the code for the phone, which was still receiving messages. Eventually officers used the telephone numbers coming in from text message to find the original owner of the cell phone.

The owner stated he left the cell phone at the gas station movie kiosk, returned and it was gone. The man was charged with petit larceny, possession of stolen property and had a warrant for his arrest for contempt of court. Bail: $1,984.

— A 69-year-old Dayton man was arrested Thursday, 4:01 p.m. in the area of Highway 50 East and Brown Street on a misdemeanor warrant for battery issued June 17, 2014 out of Elko Justice Court. Bail $1,130.

— A traffic stop Thursday night at Saliman and Robinson streets for a broken windshield led to the arrest of three men on various misdemeanor charges and warrants.

A 25-year-old Carson City man was arrested on three misdemeanor warrants, one issued August 15, 2013 out of Carson City and two from Reno Township Court. A 27-year-old man was arrested on a failure to appear warrant out of Carson City Justice Court issued April 30, 2014. A 28-year-old man was arrested on three misdemeanor charges: possession of less than an ounce of marijuana, possession of drug paraphernalia, and windshield obstruction.

— A 20-year-old Carson City man was arrested Friday, July 4, 3:01 a.m. in the 500 block of North Carson Street for being an ex-felon failing to register and being an intoxicated pedestrian. According to the arrest report the man, who was wearing shorts, was without a shirt and smelled of alcohol, was pushing a bicycle, stopped in front of and began to interrupt officers as they were dealing with an arrest next to a casino.

The man said he was upset about an officer contacting him earlier that night regarding riding a bike without a headlight. He was asked to leave as the officers were working on something else and were in the middle of an arrest. The man did not leave. He was asked for identification, which came back as him being an ex-felon for manslaughter. He told officers that he doesn't have a home and that he was "couch surfing." He was subsequently arrested and became combative with jail staff. Bail $237.

— A 26-year-old Carson City man was arrested Friday, 2:43 a.m. in the 500 block of North Carson Street on a charge of disorderly conduct. Officers responded to a casino regarding a man who refused to leave. Officers spoke with casino security who said the man started yelling "The Batmobile" loudly around the poker machines, and at himself. Deputies approached the man and told him he was too intoxicated and the business wanted him to leave.

The man said he did not want to leave. Officers attempted to call the man a cab to take him home but the man insisted on staying at the casino, the arrest report states.
Security asked the man be arrested for disorderly conduct. The man was cuffed and taken to jail. Bail: $242.

Those arrested and facing criminal charges are innocent unless proven guilty in a court of law.

UPDATE 12:42PM: Southbound I-580 in Carson City was opened as of around 11 a.m., said a Nevada State Police spokeswoman, who provided more details of the fatal crash that happened early Wednesday morning.

Both the driver of a car and a truck involved in the crash died. Their names have not been released pending notification of family members. The car, which was going the wrong way — northbound in the southbound lane — struck the truck and caught on fire, according to a NSP preliminary investigation thus far. The vehicle makes are unknown at this time.

A 37-year-old man was arrested for suspicion of obstruction, trespassing and felony sex offender failing to register, according to a Carson City Sheriff's Office booking report.
